Terms and Conditions for Dietitian Services

This website and associated services are owned and operated by Dietitian on a Mission (Adrian Griesheimer, ABN [90993469021]).

By booking an appointment or engaging services with Dietitian on a Mission, you agree to these Terms and Conditions ("Terms"). These Terms form a binding agreement.  If booking for a minor or another person, you warrant that you have the authority and that they will comply. If you disagree, do not book or use our services. We may update these Terms; continued use constitutes acceptance.

1. Cancellations and Credits Your Cancellations:

• If you cancel your appointment with more than 48 hours' notice you will be provided a Credit for your booking.

• 24-48 hours' notice: 50% of the appointment fee charged (e.g., 50% of initial or review fee).

• Less than 24 hours' notice or no-show: 100% of the appointment fee charged.

•To cancel or reschedule, contact us as soon as possible at dietitianonamission@outlook.com to notify us and free up the slot for others.

•We may waive fees at our sole discretion in exceptional circumstances (e.g., verified medical emergencies with documentation).

•Our Cancellations: If we need to cancel or reschedule (e.g., due to illness or technical issues), we will offer a full refund of prepaid fees, a credit for future services, or rescheduling. We are not responsible for any other costs or losses (e.g., travel expenses or lost time).

Refunds will be processed within 14 days. No refunds for completed services or change of mind, except as required by Australian Consumer Law (ACL).

2. Services

We provide evidence-based nutritional counselling as an Accredited Practising Dietitian, accredited by Dietitians Australia (DA). Services include assessments, plans, and education, delivered in-person or via telehealth, per DA guidelines. All sessions are conducted via telehealth unless otherwise arranged. Services are for informational purposes only and are not medical diagnosis or treatment. Consult a medical doctor for medical issues. No guarantees on outcomes; results depend on individual factors.

 

3. Bookings

No referral is needed to book, but one may be required for rebates (e.g., Medicare or private health funds). Provide accurate information (e.g., health history, referrals). To prepare for your session, please email any relevant medical documents (e.g., test results or doctor notes) to dietitianonamission@outlook.com at least 24 hours prior for optimal service. Bookings are confirmed upon payment or receipt. We may cancel or reschedule with notice.

•For telehealth: You consent to electronic delivery and must ensure a secure setup (device, internet, private space). Risks include technical failures; we use encrypted tools but cannot guarantee absolute security. Age Requirements:

• Clients aged 18 and over: You may book and attend independently.

• Clients under 18 (minors): Parental or legal guardian consent is required.

• Clients under 16: In addition to consent, a parent or legal guardian must accompany you during the appointment (in-person or telehealth).

 

4. Fees and Payments

Fees are displayed at booking and include GST. Payment is required in full upfront via specified methods. Rebates (e.g., Medicare, Private Health, NDIS, DVA): Verify your eligibility and handle paperwork (e.g., Chronic Disease Management Plan). We process claims where possible; you cover any gaps. Payments are non-refundable except as per Section 1 or ACL.

 

5.Privacy and Consent

We comply with the Privacy Act 1988, Australian Privacy Principles (APPs), and state health records laws. Our Privacy Policy is available upon request at dietitianonamission@outlook.com. You consent to:

• Secure storage of your data.

• Disclosure to your healthcare team when needed.

• Anonymised use for research or education.

You may withdraw consent anytime (subject to law). Records are retained for at least 7 years (or until age 25 for minors). We are not liable for external data breaches but will notify you as required.

 

6. Your Responsibilities

Provide accurate and complete information, including referral sources. Attend prepared and follow instructions. Respect our staff; abusive behaviour may result in service termination without refund. Seek medical advice as needed.

 

7. Standards and Disclaimers

We follow the DA Code of Conduct for ethical practice. Advice is supplementary; we are not liable for misuse or third-party services.

 

8. Liability and Indemnity

To the maximum extent permitted by law (including ACL), we exclude liability for any losses (direct, indirect, or consequential) arising from our services, including outcomes, reliance on advice, or force majeure events. Our liability is limited to the fee paid. You indemnify us against claims arising from your breach, inaccurate information, or related actions.

 

9. Complaints

Contact us first at dietitianonamission@outlook.com. If unresolved, escalate to Dietitians Australia or your state complaints body (e.g., Health Care Complaints Commission). Keep disputes confidential; breaches may lead to legal action.

 

10. General

These Terms are governed by the laws of Victoria, Melbourne, Australia.

 

Amendment of Terms

We reserve the right to change, modify, add, or remove portions of these Terms at any time. Please check these Terms regularly prior to using our services or website to ensure you are aware of any changes. We will endeavour to highlight any significant or substantive changes to you where possible. Your continued use constitutes acceptance.
